Project intake form template

A fill-in template that captures everything Chykalophia needs to kick off your project — copy it and fill in what you know.

getting-startedtemplatesbeginner

Before we can start planning your project, we need to understand your business, your goals, and the scope of what you're asking for. This form collects all of that in one place.

You don't need to have all the answers. Fill in what you know, write "unsure" or "let's discuss" where you're not certain, and we'll cover the gaps in our kickoff call.

Quick summary

Copy this form into a Google Doc or paste it into a ClickUp comment. Fill in as much as you can — even partial answers help us prepare. Send it to support@chykalophia.com or attach it to your project task before your first call.

What you'll need

Beginner 20–40 minutes

You don't need any technical knowledge to complete this form. Have your business details, any existing website URLs, and a rough idea of your goals handy.

How to use this template

Copy the template. Select all the text in the "Template" section below and paste it into a Google Doc, a Word document, or a ClickUp task comment.

Fill in your answers. Replace every [Your answer here] with your actual answer. Write in plain language — no need to be formal.

Leave notes where you're unsure. If you don't know the answer, write "unsure" or "let's discuss." Blank fields slow us down more than honest uncertainty.

Send it to us. Email it to support@chykalophia.com, or drop it into your ClickUp project task as an attachment or comment.

Why each section matters

About your business

We do better work when we understand who you are and who you serve. This section helps us write better copy, make better design decisions, and ask smarter questions.

About this project

A clear goal prevents scope creep and mis-set expectations. Knowing your deadline and budget up front means we can tell you honestly whether the timeline is realistic.

Existing digital presence

We need to know what already exists before we can plan what to build. Surprises during discovery slow everything down.

Brand & content

If brand and content gaps exist, they belong in the project scope. Knowing this early prevents the most common project delay: waiting for client content.

Technical & integrations

Some integrations take significant time. Discovering them late can add cost and delay a launch. Better to surface them now.

Stakeholders & process

Projects slow down when the approval chain is unclear. Knowing who has final say up front keeps decisions moving.

Reference & inspiration

You can't always describe what you want in words — but you can point to it. These links give us a visual vocabulary we can work with together.
